Job Title: Clinical Home Health Intake Coordinator

Overview:

Fortis Health, a leading provider of home health services, is seeking a highly skilled Clinical Home Health Intake Coordinator to join our team. As a key member of our intake team, you will be responsible for efficiently processing incoming referrals, ensuring compliance with regulations, and promoting high-quality patient care.

Responsibilities:
  • Process incoming referrals for all branches, ensuring timely and accurate intake.
  • Understand and comply with regulations and requirements for admission to home health services.
  • Communicate effectively with physicians, patients, and sales and marketing staff to ensure seamless referral processing.
  • Coordinate and confirm valid Face to Face visits and attestation forms for each referral.
  • Promote compliance with incoming documentation and maintain accurate records.
  • Monitor admission medicare to private insurance percentages and run intake reports as needed.
  • Maintain a referral tracking sheet and demonstrate a desire to set and meet objectives.
  • Perform job duties in compliance with agency policies and procedures, as well as community and professional standards.
  • Communicate necessary information to the supervisor and management team to ensure coordination of services and activities.
Qualifications:
  • Current LPN licensure in the state of Indiana.
  • Business and professional setting experience.
  • Healthcare experience preferred.
  • Excellent written and verbal communication skills.
  • Ability to speak, read, write, and comprehend English.
  • Ability to handle multiple tasks simultaneously and meet deadlines.
  • Excellent organizational and time management skills.
  • Self-starter.
