Job Title: Clinical Home Health Intake Coordinator

Overview:

Fortis Health is seeking a skilled Clinical Home Health Intake Coordinator to join our team. As a Clinical Home Health Intake Coordinator, you will be responsible for completing the full intake process, including inputting patients in HCHB, confirming physicians, and verifying F2F visits.

Responsibilities:
  • Efficiently process incoming referrals for all branches.
  • Understand and comply with regulations and requirements for admission to Home Health.
  • Communicate with physicians and patients as needed.
  • Coordinate and confirm valid Face to Face visits and attestation forms for each referral.
  • Promote compliance with incoming documentation.
  • Monitor current admission Medicare to private insurance percentages.
  • Run and maintain intake reports within HCHB.
  • Maintain a referral tracking sheet.
  • Demonstrate a desire to set and meet objectives and find increasingly efficient ways to perform tasks.
  • Perform job in compliance with agency policies and procedures as well as community and professional standards.
  • Communicate necessary information to the supervisor and management team to ensure coordination of services and activities.
Qualifications:
  • Current LPN licensure in the state of Indiana.
  • Business and professional setting experience.
  • Healthcare experience preferred.
  • Excellent written and verbal communication skills.
  • Ability to speak, read, write, and comprehend English.
  • Ability to handle multiple tasks simultaneously and meet deadlines.
  • Excellent organizational and time management skills.
  • Self-starter.
