POSITION SUMMARY/RESPONSIBILITIES
The Service Coordinator - Licensed Vocational Nurse (SC-LVN) works in collaboration with medical, behavioral, and social support professionals within the Service Coordination team. This role is essential in delivering a comprehensive assessment of STAR+PLUS Members' unique circumstances, requirements, and preferences while providing essential education and health-related information to the Member, their Representative, and Support Network.

The SC-LVN is responsible for coordinating both covered and uncovered Long Term Services and Supports (LTSS) for STAR+PLUS Members, ensuring that the Members' physical, behavioral, functional, and psychosocial needs are effectively met.

This position adheres to the policies, procedures, and protocols of Community First Health Plans, Inc. (Community First) to foster and maintain positive working relationships with colleagues, Members, and visitors.

EDUCATION/EXPERIENCE
A minimum of three years of clinical nursing experience is preferred. Additionally, at least two years of experience in a managed care setting or working with individuals with disabilities and vulnerable populations facing chronic or complex health conditions is preferred. Proficiency in data entry is required, and call center experience is advantageous. Familiarity with RUG/MDS/PASRR is also preferred.

LICENSURE
A current unrestricted license as a Licensed Vocational Nurse in the State of Texas is mandatory.
