At Ennoble Care, we are committed to providing compassionate and personalized care to patients in need. We are currently seeking a skilled Hospice Provider to join our team in Salem, NJ.

About the Opportunity

This is a part-time PRN opportunity that offers flexibility and autonomy. As a Hospice Provider, you will be responsible for examining patients, assessing their health, and creating personalized care plans.

Key Responsibilities
  • Provide high-quality care to primary and palliative patients in the Salem, NJ area
  • Assess patient's health by performing physical examinations and updating medical histories
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Develop impressions, diagnosis, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
Requirements
  • Board Certified, with a valid NP certification in the state of New Jersey
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of Hospice care experience with geriatric patients (hospice experience highly preferred but not required)
