Palliative Care Nurse

We are seeking a compassionate and skilled Palliative Care Nurse to join our team in Salem, OR. As a Hospice RN Case Manager, you will play a vital role in providing high-quality patient care and support to patients and their families.
About the RoleThis is a full-time opportunity that involves coordinating the care of hospice patients to meet their holistic needs. You will work closely with patients, families, and healthcare teams to develop and implement personalized plans of care.
Responsibilities- Use your teaching skills to empower family caregivers and help them identify options for care to optimize quality of life.
- Coordinate patient care to meet the holistic needs of patients and their families/caregivers.
- Provide caring, professional nursing services to hospice patients.
- Perform initial and ongoing assessments of patients, developing and revising plans of care as needed.
- Document assessments, planning, and interventions in a timely manner.
- Current Oregon RN license.
- At least 1 year of recent clinical experience as an RN.
- Certification as a Hospice and Palliative Care Nurse (preferred).
- Fully vaccinated against COVID-19.
- Current CPR certification.
- Willingness to work in various patient settings, including care facilities, hospitals, and patient homes.
- Computer competency and EMR experience.
