Ennoble Care is a leading provider of mobile primary care, palliative care, and hospice services. Our team of clinicians delivers high-quality patient care in the comfort of patients' homes across New York, New Jersey, Maryland, DC, Virginia, and Georgia.
We are seeking an experienced Hospice Provider to join our team in Salem, NJ. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of patients and their families.
Responsibilities- Deliver high-quality patient care in a home-based setting
- Conduct thorough assessments and create personalized care plans
- Foster strong relationships with patients, families, and caregivers
- Collaborate with office staff and clinical operations to ensure seamless care coordination
- Board Certified Nurse Practitioner (NP) with valid certification in New Jersey
- Strong clinical skills and ability to work independently
- Positive attitude and excellent communication skills
- 1-2 years of experience in hospice care, preferably with geriatric patients
- A competitive salary range: $120,000 - $150,000 per year (depending on experience)
