Responsibilities:

  • Provide high-quality home health care services to clients in their homes
  • Assess client needs and develop personalized care plans
  • Administer medications, perform wound care, and provide other medical treatments as needed
  • Monitor client progress and report changes to the clinical team
  • Collaborate with other healthcare professionals to ensure comprehensive care

Qualifications:

  • Current RN license in Pennsylvania
  • Minimum one year of recent, verifiable clinical experience
  • Prior home care experience preferred, but not required
  • Ability to work independently and manage time effectively
  • Strong interpersonal and communication skills
