Pinnacle Home Care Inc. Seeks Dedicated Speech Therapist PRN

Pinnacle Home Care Inc., a leading provider of home health care services, is seeking a skilled Speech Therapist PRN to join our team. As a Speech Therapist PRN, you will play a vital role in providing high-quality speech therapy services to our patients in their homes.

Responsibilities:
  • Evaluate patients' speech and language abilities and develop personalized treatment plans to enhance their communication skills.
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments, to identify areas for improvement.
  • Establish and implement speech therapy treatment plans in consultation with physicians and revise them as necessary to ensure optimal patient outcomes.
  • Collaborate with other healthcare professionals to develop and implement interdisciplinary care plans that address patients' speech and language needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
  • Document patient progress and treatment outcomes in a timely and accurate manner to inform future care decisions.
Requirements:
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
