Job Summary

Pinnacle Home Care Inc. is seeking a skilled Speech Therapist to join our team of healthcare professionals. As a Speech Therapist, you will provide high-quality speech therapy services to patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Evaluate patients' speech and language abilities, identifying areas for improvement and developing personalized treatment plans.
  • Conduct comprehensive assessments, including Outcome and Assessment Information Set (OASIS) assessments, to inform treatment decisions.
  • Establish and implement speech therapy treatment plans in consultation with physicians, revising as necessary to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Teach patients and their families speech pathology techniques to promote independence and self-care.
  • Record and report patient progress, including reaction to treatment and any changes in condition, to physicians and other healthcare professionals.
  • Participate in the development and revision of patient care plans, ensuring alignment with physician orders and patient needs.
Requirements
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Ability to work with minimal supervision, demonstrating good observational, judgment, and communication skills.
