Speech Therapist PRN Job Description

Pinnacle Home Care Inc. is seeking a skilled Speech Therapist PRN to provide high-quality speech therapy services to patients in their homes. As a Speech Therapist PRN, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Evaluate patients' communication and swallowing abilities and develop effective treatment plans to improve their functional abilities.
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments, to identify areas for improvement.
  • Establish and maintain effective communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Provide education and training to patients and their families on speech and language skills, as well as strategies for improving communication and swallowing abilities.
  • Collaborate with other healthcare professionals to develop and implement comprehensive care plans that address patients' physical, emotional, and social needs.
  • Document patient progress and outcomes in a timely and accurate manner, using standardized assessment tools and reporting procedures.
Requirements:
  • Master's degree in Speech-Language Pathology or equivalent.
  • Current licensure as a Speech Therapist in the State of Florida.
  • At least two years of experience as a Speech Therapist in a home health care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
