About the Role

We are seeking a highly motivated and compassionate Peer Support Specialist to join our team at Central City Health. As a key member of our integrated care team, you will provide peer support services to clients experiencing mental illness, chronic health conditions, and/or homelessness.

Job Description

The Peer Support Specialist is responsible for conducting outreach services, targeting individuals in need of a medical home and/or with chronic disease. You will assist patients in gaining access to and navigating primary health care medical homes and other community-based social services. Your goal will be to facilitate communication with the treatment team to determine steps necessary to achieve patient goals.

Requirements

To be successful in this role, you will need to possess:

  • A High School Diploma or GED equivalent.
  • Peer Support Specialist Certification.
  • A current Michigan Chauffeur's Driver's License.
  • Demonstrated ability to adhere to State of Michigan driving laws.
  • Experience working with patients exhibiting severe and persistent mental illness (preferable).
