About the Job

We are seeking a highly motivated Certified Peer Support Specialist to join our team. As a community health advocate, you will work collaboratively with patients experiencing mental illness, chronic health conditions, and/or homelessness to provide peer support services and help them regain independence within the community.

Job Responsibilities
  • Conduct outreach services to target individuals in need of medical care and/or chronic disease management.
  • Assist patients in gaining access to primary healthcare services and navigating community-based social services.
  • Provide recovery education to service recipients for every phase of the recovery journey.
  • Link individuals to appropriate resources when needed and provide vision-driven hope and encouragement.
  • Teach problem-solving techniques and model effective coping strategies.
  • Maintain compliance with productivity requirements and participate in performance improvement activities.
Requirements
  • High School Diploma or GED equivalent required.
  • Peer Support Specialist Certification required.
  • Current Michigan Chauffeur's Driver's License required.
  • Demonstrated ability to adhere to State of Michigan driving laws.

  • Estimated Salary: $40,000-$60,000 per year based on experience.
