Position Overview:

Cambay Healthcare LLC is seeking a skilled and compassionate Hospice Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and families in their homes and communities.

Key Responsibilities:

  • Plan, organize, and direct hospice care services, with a focus on community health education and experience.
  • Build relationships with community resources to develop and implement patient-centered care plans.
  • Coordinate patient care for assigned caseloads, including communication with physicians and other healthcare professionals.
  • Teach patients and families/caregivers self-care techniques and provide medication, diet, and other instructions as ordered by physicians.
  • Work collaboratively with the interdisciplinary team to ensure seamless patient care.

Qualifications:

  • Graduate of an accredited school of nursing with one to two years of recent acute care experience and one to two years of recent experience in home care.
  • Current licensure in the State and BLS Hands-On CPR certification.
  • Bachelor's degree with one year of hospice care experience preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
