Job Summary

We are seeking a skilled and compassionate Hospice RN to join our team. As a key member of our interdisciplinary care team, you will provide direct patient care services, educate patients and their families on care and management, and collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes. This role requires strong communication skills, empathy, and the ability to work effectively in a fast-paced environment.

Key Responsibilities:

* Provide direct patient care services, including assessments, medication management, and wound care
* Educate patients and their families on disease management, self-care techniques, and end-of-life care options
* Collaborate with other healthcare professionals to develop and implement individualized care plans
* Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed
* Communicate effectively with patients, families, and caregivers to ensure understanding and cooperation

Requirements:

To be successful in this role, you must possess a valid RN license in the state where you plan to work (or eligibility to obtain), at least one year of nursing experience in hospice, home health, ICU, geriatric, or oncology, and excellent communication and interpersonal skills. A current driver's license and automobile insurance are also required. Additional experience in hospital settings, critical care, or emergency medicine is preferred.
