Job Summary

We are seeking a compassionate and dedicated Per Diem LVN to provide high-quality patient care in a flexible and supportive environment.

About the Role

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Key responsibilities include following the plan of care, administering treatments, and collaborating with the healthcare team to achieve positive patient outcomes.

Key Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Document patient services accurately and timely to reflect the plan of care.
  • Assess and educate patients and their families/caregivers on pertinent diagnosis and self-care plans.
  • Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group.
  • Utilize equipment and supplies efficiently.
  • Manage pain and symptoms effectively, evaluating patient responses to treatments/medications.
  • Participate in personal and professional growth and development, quality assessment performance improvement teams, and activities.
Requirements
  1. Graduate of an accredited practical nurse or vocational nursing program.
  2. One year of nursing experience; community health/hospice or medical/surgical experience preferred.
  3. Currently licensed as an LPN/LVN in the state.
  4. Comply with accepted professional standards and practice.
  5. Demonstrate excellent verbal and written communication, and organizational skills.
  6. Possess and maintain current CPR certification.
