Position Overview
Phoenix Senior Living is in search of a dedicated Supply Chain Coordinator and Resident Transport Specialist to join our team.

Work Schedule:

Monday - Friday; 40 hours per week

ROLE SUMMARY:

The Supply Chain Coordinator collaborates closely with the Nursing Director and Business Office Manager to ensure the efficient receipt, distribution, and accurate billing of supplies. This role also involves the safe transportation of residents to various activities and appointments.



KEY RESPONSIBILITIES:

Supply Chain Coordination:

  • Utilize the DSSI e-procurement platform and maintain necessary spreadsheets for integration into the YARDI system.
  • Ensure accurate entry of all resident charges into the YARDI system.
  • Conduct daily and weekly scanning to ensure all records are verified by the end of the month.
  • Complete the monthly spend down report and document all purchases for budget tracking.
  • Coordinate the ordering of all household and chargeable supplies in collaboration with nursing and other staff.
  • Ensure that all costs are accurately linked to charges.
  • Oversee the supply check-in process upon delivery and verify against purchase orders.
  • Monitor and analyze supply usage trends.
  • Conduct daily and monthly physical inventory checks and reconciliations.
  • Exhibit knowledge and respect for the rights, dignity, and individuality of each resident in all interactions.
  • Understand emergency and disaster protocols of the facility.
  • Demonstrate respect for colleagues and respond to resident needs in accordance with facility policies.
  • Exhibit strong multitasking abilities and manage stress effectively.
  • Adhere to all safety protocols when handling supplies and equipment.
  • Fulfill essential job functions with or without reasonable accommodation.
  • Maintain confidentiality of resident information at all times.
  • Ensure supply and storage areas are organized, clean, and safe.
  • Deliver emergency supplies and equipment as needed.
  • Perform additional duties as assigned.

Transportation Duties:

  • Conduct daily safety inspections of the vehicle.
  • Report any vehicle, routing, or scheduling issues to the Executive Director immediately.
  • Provide safe assistance and transportation for residents.
  • Maintain cleanliness of the interior of company vehicles.
  • Perform daily fueling and weekly fluid checks on the vehicle.
  • Exhibit patience and tact when interacting with residents.
  • Demonstrate reliability and the ability to work independently, remaining calm in emergencies.
  • Complete required driver training to ensure safety and regulatory compliance.

Qualifications:

  • Familiarity with regulatory standards and compliance requirements.
  • Proficient in using personal computers and relevant software applications (word processing, spreadsheets, etc.).
  • Strong organizational, analytical, and communication skills.
  • Possess a valid Driver's License.
  • Previous experience in transporting elderly or physically/mentally challenged individuals is preferred.

Education and Experience:

  • High school diploma or equivalent is required; SRNA certification is preferred.
