Key Responsibilities

The Transplant Pharmacy Specialist will:

  • Deliver expert consultation and guidance on medication management for solid organ transplant patients.
  • Collaborate with a multidisciplinary team to ensure comprehensive patient care in both inpatient and outpatient settings.
  • Enhance patient outcomes through evidence-based, patient-focused pharmacotherapy while monitoring for medication-related issues and cost efficiency.
  • Adhere to regulatory standards and local quality initiatives to ensure safe medication practices and promote effective medication management.
  • Provide pharmacokinetic consultations as necessary.
  • Identify, track, document, and report adverse drug reactions and medication errors.
  • Evaluate drug therapy for appropriateness, dosage, and potential interactions.
  • Communicate ongoing patient information and treatment efficacy to all healthcare providers involved.
  • Conduct educational sessions for the transplant team and clinic staff, and provide counseling to patients and caregivers regarding medication therapy.
  • Participate in interdisciplinary committees, protocol development, and the education of healthcare professionals.
  • Coordinate patient care issues and therapy recommendations with pharmacy personnel.
  • Support research initiatives within the specialty team as needed.
  • Engage actively in specialty team meetings, including discharge planning and educational conferences.
  • Stay updated on evolving protocols and clinical pharmacy standards relevant to solid organ transplantation.
Qualifications

Applicants should possess:

  • A degree from a pharmacy program accredited by the Accreditation Council for Pharmacy Education.
  • Completion of a Post-Year Graduate 2 (PGY2) residency in a specialty area and board certification within 12 months of hire; OR a Post-Year Graduate 1 (PGY1) residency with at least two years of relevant experience and current board certification; OR a minimum of four years of relevant experience with current board certification if no residency training is completed.
  • Preferred experience in specialty areas such as Transplant, Infectious Disease, or Hematology/Oncology.
  • Knowledge of regulations governing pharmacy practices in acute care settings.
  • Strong communication skills, both verbal and written, for effective interaction with patients and healthcare professionals.
  • Proficiency in applying professional pharmacy methods and techniques.
  • A current license from the state pharmacy board.
  • Active board certification in the relevant specialty area.
Compensation and Benefits

Work Schedule

This is a full-time position with a Monday to Friday schedule.
