Adecco is currently assisting a local client in recruiting for a
Case Manager for Community Outreach. This role offers a Direct Hire opportunity with a competitive pay rate of $21.00 per hour.
Shift:
Sunday-Thursday 7am-3:30pm. Days and hours can be flexible.
Summary:
The Community Outreach Case Manager plays a vital role in supporting adult clients requiring intensive services, particularly those who are chronically homeless and facing various health challenges, including potential substance use issues.
This position involves a blend of street outreach and community engagement, including home visits.The role is designed to foster a structured, safe, and supportive environment that aligns with the mission, vision, and values of the organization.
Duties & Responsibilities
- Manage a caseload of clients while ensuring timely and accurate documentation.
- Identify strengths and barriers for each client, completing outreach case management assessments promptly.
- Establish and nurture rapport with each client.
- Assist clients in meeting their basic needs.
- Evaluate and monitor progress toward desired outcomes through assessments and consistent client contact.
- Help clients secure and maintain housing.
- Conduct street outreach within the community as designated.
- Complete Coordinated Entry assessments and referrals as necessary.
- Address daily crises as they arise.
- Refer clients to suitable internal and external service providers.
- Network with relevant individuals, agencies, and organizations within the community.
- Adhere to program policies and procedures.
- Maintain professional boundaries and confidentiality with clients, following a strict 'need to know' policy regarding sensitive information.
Education Requirements
An Associate's Degree in human services or a related field, or equivalent work experience is required. A Bachelor's Degree in social work or a related field is preferred.

Knowledge, Skills, & Abilities Needed to Perform Effectively in this Position:
Ability to demonstrate sensitivity and compassion toward a vulnerable population.
Ability to maintain a calm and professional demeanor while managing daily stress from working with individuals facing complex issues.
Ability to communicate effectively, manage time efficiently, and utilize computer skills proficiently.
Familiarity with community services in the local area is preferred.

Pay Details: $21.00 per hour
