Adecco is currently assisting a local client in recruiting for a
Outreach Case Manager. This position is a Direct Hire opportunity, offering a competitive pay rate of $21.00 per hour.

Shift:

Sunday-Thursday 7am-3:30pm; Days/hours can be flexible.

Position Overview:

The Outreach Case Manager plays a crucial role in providing support to adult clients requiring intensive services, particularly those facing chronic homelessness and various health challenges, including potential substance use issues.

This role involves a mix of community outreach and client meetings, including home visits.

The individual will help maintain a structured, safe, and supportive environment that aligns with the mission, vision, and values of the organization.



Key Responsibilities:

  • Manage a caseload of clients while ensuring timely and accurate documentation.
  • Identify strengths and barriers for each client and conduct outreach case management assessments promptly.
  • Establish and nurture rapport with each client, providing supportive counseling to encourage positive growth.

Additional responsibilities include:

  • Assisting clients in meeting their basic needs.
  • Monitoring progress towards desired outcomes through consistent contact and assessment.
  • Helping clients secure and maintain housing.
  • Conducting community outreach in accordance with organizational guidelines.
  • Completing necessary assessments and referrals.
  • Addressing daily crises as they arise.
  • Connecting clients with appropriate internal and external service providers.
  • Networking with relevant individuals, agencies, and organizations in the community.
  • Adhering to organizational policies and procedures.
  • Maintaining professional boundaries and confidentiality in client interactions.

Education Requirements:
Associates Degree in human services or a related field, or equivalent work experience. A Bachelor's Degree in social work or a related field is preferred.

Qualifications:
To excel in this role, candidates should possess the following skills and abilities:
  • Demonstrated sensitivity and compassion towards vulnerable populations.
  • Ability to maintain a calm and professional demeanor while managing daily stressors.
  • Effective communication skills, time management, and proficiency in computer use.
  • Familiarity with community services in the area is preferred.

Compensation:
$21.00 per hour
