CTI Support Specialist. This Case Manager position offers a Temp to Hire opportunity, with a competitive pay rate of $21.00 per hour.

Shift:


Tuesday-Saturday 10:30am-7am (some flexibility)

The role of the SOS CTI Care Manager involves engaging with the community, addressing members' needs before and after their transition from homelessness to stable housing, enhancing their daily living skills, accompanying them to appointments, and advocating for them in instances of discrimination or healthcare disparities.

Key principles such as member choice, harm reduction, non-coercion, flexibility, and person-centered care are fundamental to the SOS program model and should be prioritized in the care provided by the Care Manager.

This position operates with considerable independence in the field, with consultation available from the Team Lead as necessary.

Duties & Responsibilities:


  • Engage in persistent and assertive outreach utilizing strength-based approaches, starting from known community gathering spots or during hospital admissions or emergency visits.
  • Continuously evaluate the health and social needs of participants through conversational and observational assessments and formalized risk assessment tools for those identified as high risk.
  • Participate in discharge planning meetings to identify optimal community resources for returning members.
  • Assist with navigating appointments, including accompanying members, providing transportation training, re-engaging in community care, and addressing barriers to care.
  • Conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing, and other social needs of members within the community.
  • Gather historical and collateral information from various sources to support members' behavioral and physical health needs.
  • Monitor, evaluate, and document participants' progress concerning care plan goals.
  • Adhere to professional boundaries and protocols as established by Monroe Plan.
  • Comply with program documentation requirements in the Electronic Health Record.
  • Collaborate with regional partners to identify available housing and support participants throughout the process.
Tasks may include applying for housing, preparing for interviews, following up with housing providers, and assisting with moving in and obtaining necessary supplies while familiarizing members with their new neighborhoods.

  • Once housed, collaborate with members and their housing providers to resolve clinical issues affecting their ability to manage and maintain supportive housing.
  • Build relationships with community providers to ensure members are connected with appropriate services as they reintegrate into the community.
Document a Person-Centered Care Plan in collaboration with clients and providers.

  • Collect and report data as required, working with the team leader and other SOS staff to utilize data for informing future care delivery.
  • Provide program information to members, providers, and other organizations as requested to promote and support program participation.
  • Contribute to team efforts by accomplishing assigned tasks.

Knowledge, Skills, & Abilities Needed to Perform Effectively in this Position

  • Strong oral and written communication skills.
  • Detail-oriented and organized; must possess exceptional time management skills.
  • Knowledge of resources for the homeless, shelter systems, and transportation options.
  • Understanding of counseling principles and methods for mental health and substance use disorders.
  • Familiarity with treatment, rehabilitation, and community support programs relevant to recipients, families, and staff.
  • Ability to develop, evaluate, implement, and adjust treatment interventions to meet individual needs.
  • Capability to prepare accurate and timely reports.
  • Demonstrates respect for individual and family diversity while maintaining confidentiality.
  • Ability to work collaboratively as a team member.
  • Proficient in computer usage.
Must be able to pass computer documentation competency testing for all software platforms utilized within the program.

  • Present in a professional and articulate manner that fosters the development of therapeutic relationships with members and community providers.
  • Provide feedback to providers regarding members' progress and any barriers encountered.
  • Exhibit strong listening skills to enhance member engagement and the development of a person-centered care plan.
  • Demonstrate a passion for the mission and vision of the organization.

  • Valid NYS Driver's License in compliance with company policy.
  • Must adhere to CDC guidelines regarding Covid-19, including wearing a mask while working.
  • Full vaccination against Covid-19, including the Booster (when eligible), is strongly encouraged.

Pay Details: $21.00 per hour
