Key Responsibilities
  • Community Outreach and Engagement: Develop and maintain relationships with individuals experiencing homelessness, providing them with information and support to access vital services.
  • Assessment and Planning: Conduct comprehensive assessments to identify the needs of individuals, develop personalized care plans, and coordinate services to support their well-being.
  • Service Coordination: Collaborate with community providers, healthcare organizations, and government agencies to ensure individuals receive the necessary services and support.
  • Advocacy and Support: Provide advocacy and support to individuals experiencing homelessness, addressing barriers to care and promoting their overall well-being.
  • Documentation and Reporting: Maintain accurate and timely records of individual interactions, service coordination, and outcomes.
Requirements
  • Education and Experience: Bachelor's degree in a related field (e.g., social work, psychology, sociology) and a minimum of two years of experience in care management or a related field.
  • Skills and Qualities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and a strong commitment to serving individuals experiencing homelessness.
