Job Summary

The Support Coordinator/Service Coordinator/Care Manager is responsible for providing case management activities to meet the needs of clients assigned to the consolidated care team. This role involves developing individualized care management plans, implementing care plans through brokering and coordinating services, and monitoring and evaluating clinical outcomes to ensure services are provided in a professional, comprehensive, and cost-effective manner.

The Support Coordinator/Service Coordinator/Care Manager will assist clients in identifying and communicating their requests and needs for services and supports, which may include housing, medical, social, vocational, and other community services.


Key Responsibilities

  • Complete assessments to identify client needs and desires for service delivery.
  • Develop and modify Person-Centered Service Plans for each client at least annually or when there is a significant medical or social change.
  • Ensure client choice of providers by providing information for clients to make informed decisions.
  • Ensure Behavioral Support Plans and Crisis Plans are consistent with Individual Support Plans.
  • Report incidents of client abuse, neglect, or exploitation immediately to supervisor/director.
  • Actively coordinate with other individuals and/or entities essential in physical and/or behavioral care delivery for clients.
  • Research and develop alternative solutions to client needs.
  • Ensure confidentiality regarding sensitive material and private health information of each individual served.
  • Attend required trainings and recertification classes.
  • Confer regularly with supervisor/director to understand and consider factors such as client needs, abilities, learning levels, and physical limitations.
  • 50% travel to clients as needed.

Requirements

  • BS/BA degree in related coursework.
  • Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
  • Experience with Microsoft Office and state-maintained databases.
  • Maintain proficiency in company-sponsored training and certifications.
  • Maintain CPR/First Aid certification and updated state-related Clearances.
  • Successfully complete and maintain training courses as required or amended by program regulations.
  • Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
