The Communication Specialist in Home Health is tasked with assessing and evaluating patient care needs related to speech and language challenges, as well as providing functional training in communication, swallowing, and cognitive skills. Following this assessment, the specialist collaborates to create a tailored treatment plan, implements interventions to improve the patient's overall well-being, and monitors the patient's progress.

  • Delivers services within the defined scope of practice as per state regulations, adhering to the care plan and collaborating with other healthcare professionals.
  • Assesses the patient's functional abilities through diagnostic evaluations and assists in formulating the therapy care plan.
  • Facilitates effective communication for patients, enabling them to express their thoughts in accordance with their individual conditions and established standards.
  • Records and reports the patient's responses to treatment and any changes in their condition to the supervising nurse or physician.
  • Educates the patient, their family, and other healthcare team members on aspects of speech therapy in which they can engage.

Licensure Requirements

  • Must hold a valid license in Speech Therapy in the state of Idaho
  • Current CPR certification is required
  • Must possess a valid driver's license and vehicle insurance, along with access to reliable transportation
