Job Summary

We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our Patient Support Center team at Texas Health Action. As a Patient Support Center LVN, you will play a critical role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Interpret lab results, send medication refills, and schedule treatments in accordance with our organization's clinical standards and procedures.
  • Ensure accurate and timely documentation of patient consultations and treatments in their medical records.
  • Provide patient education on disease processes, therapies, and healthful behaviors.
  • Communicate effectively with staff, consultants, patients, partners, and community members.
  • Participate in staff meetings, planning sessions, and in-service training as needed.
  • Assist with medication prior authorizations and perform other duties as assigned.
Requirements
  • Current LVN licensure and 2+ years of patient care experience in an outpatient clinic setting.
  • Telephony patient care experience and bilingual English/Spanish skills are highly desirable.
  • Experience working with diverse populations, including LGBTQ+ and other marginalized communities, is preferred.
