Pharmaceutical Care Specialist

JOB SUMMARY:
The Clinical Staff Pharmacist is responsible for delivering comprehensive pharmaceutical care to patients and healthcare professionals through integrated medication distribution and clinical services.
EDUCATIONAL REQUIREMENTS:
A Doctor of Pharmacy or a Bachelor of Science in Pharmacy from an accredited institution is required, along with current state licensure or eligibility for licensure to practice pharmacy as per the Texas State Board of Pharmacy.
This role is part-time, offering up to 19.99 hours per week, and is not eligible for benefits.
KEY RESPONSIBILITIES:
1. Interprets physician prescriptions for medications, ensuring that therapies are suitable for the patient's diagnosis, weight, and age (including pediatric, neonate, adolescent, adult, and geriatric populations) as detailed in the accompanying documentation.
2. Compounds and dispenses medications as prescribed by physicians across all patient care settings, adhering to departmental protocols and relevant regulations.
3. Engages in direct patient interactions while dispensing ambulatory prescriptions, ensuring compliance with all regulatory standards.
4. Identifies, prevents, assesses, and reports Adverse Drug Events when necessary.
5. Maintains comprehensive patient medication records and formulates individualized patient care plans.
6. Provides counseling to patients regarding their medications and offers drug information to the healthcare team.
7. Oversees the activities of support staff and contributes to the training and development of newly hired departmental personnel.
8. Acts as a mentor to pharmacy technicians, students, residents, and interns.
9. Participates in ongoing pharmaceutical education in line with departmental performance standards.
10. Complies with internal controls established for the department.
