Community Outreach Specialist Job Description

As a Community Outreach Specialist, you will play a vital role in connecting individuals experiencing homelessness with essential resources and services. You'll build strong relationships, understand client needs, and develop effective case plans to promote positive outcomes.

Key Responsibilities
  • Collaborate with health homes, crisis teams, and other organizations to deliver comprehensive support.
  • Develop and implement individualized case plans, ensuring seamless care coordination.
  • Establish and maintain relationships with clients, promoting trust and understanding.

Requirements:

  • Bachelor's Degree in Psychology, Social Work, or related field preferred.
  • Experience as a Case Manager or working with adults/children in an outpatient setting.
  • Current CO Driver's License and safe driving record required.
