About the Role
We are seeking a highly motivated and organized Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring that our clients receive high-quality, coordinated care. You will work closely with clients, families, and healthcare providers to develop and implement individualized care plans that meet the unique needs of each client.

Responsibilities
* Develop and implement individualized care plans for clients* Coordinate care with healthcare providers and other stakeholders* Conduct assessments and make recommendations for treatment* Provide education and support to clients and their families* Collaborate with other healthcare professionals to ensure seamless care* Maintain accurate records and reports* Participate in quality improvement initiatives*

Requirements
* Bachelor's degree in psychology, social work, or related field* One year of experience in a behavioral health setting* Excellent communication and interpersonal skills* Ability to work in a fast-paced environment* Strong organizational and time management skills* Ability to maintain confidentiality and handle sensitive information* Physical demands include sitting, standing, walking, and occasional lifting of up to 25 pounds* Must have a current Colorado driver's license and a safe driving record
