Role Summary:
We are currently seeking a Bilingual (Spanish) Case Management Specialist to engage with youth and families, utilizing their expertise to ensure adherence to program requirements.
Key Responsibilities:
- Lead and support the program structure, fostering a positive environment for all participants.
- Empower youth and families through proactive engagement and support.
- Maintain accurate and secure client files, records, and individual Service Plans.
- Participate in live supervision sessions with clients and their families.
- Regularly meet with the Program Manager for guidance and supervision.
- Actively contribute to weekly staff meetings, demonstrating leadership and engagement.
- Assist clients with discharge and aftercare planning as they transition to their next steps.
- Provide timely and accurate documentation of services, including progress reports and urgent issues.
- Oversee client assessments and develop individualized treatment plans.
- Perform additional duties as assigned.
Qualifications:
- A Bachelor's Degree is required; a Master's Degree is preferred or in progress in Social Work, Case Management, Counseling, or a related field.
- A minimum of 5 years of experience in the human services sector, particularly with adolescents and young adults.
- In-depth knowledge of homelessness-related issues.
- Proven ability to collaborate with community leaders to secure program funding and effectively manage complex projects.
