Job Title: Social Services Coordinator

Four Seasons Retirement Center is seeking a dedicated and compassionate Social Services Coordinator to join our team. As a key member of our care team, you will play a vital role in ensuring the well-being and quality of life for our residents.

Responsibilities:
  • Implement the admission process for new residents, ensuring a smooth transition to our community.
  • Develop and update resident care plans, taking into account their physical and psychological needs.
  • Provide direct counseling and support to residents, family members, and representatives, promoting emotional well-being and stability.
  • Collaborate with our healthcare team to identify and address resident needs, ensuring comprehensive care and support.
  • Assist residents in adjusting to our community, promoting continuity of relationships with family and friends.
  • Coordinate relocation planning, advising and referring residents to community resources as needed.
  • Establish and maintain confidentiality, safeguarding resident information and respecting their privacy.
  • Participate in quality assurance processes, ensuring the highest standards of care and service.
  • Educate staff on the psychosocial and behavioral needs of residents, promoting a culture of understanding and empathy.
Requirements:
  • A high school diploma or GED equivalent, with completion of a 20-hour social service course or equivalent.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility and adaptability, with a willingness to learn and grow with our team.
