Overview: Deliver comprehensive case management support to individuals residing in the emergency shelter. Develop personalized service plans aimed at promoting self-sufficiency and facilitating transitions to stable housing solutions.
Key Responsibilities:
- Administer case management services within the emergency shelter environment.
- Collaborate with clients to create tailored service plans.
- Provide ongoing support and follow-up as clients engage with new services.
- Supply information and referrals to community resources.
- Ensure accurate maintenance of client records and documentation.
- Engage in team meetings and maintain effective communication.
- Uphold client confidentiality at all times.
- Be prepared to cover shifts during emergencies.
Qualifications and Skills:
- Bachelor's degree or a minimum of three years of relevant experience.
- Awareness and sensitivity to diverse low-income populations.
- Excellent listening and communication abilities.
- Capability to work autonomously.
- Bilingual skills are advantageous.
- Possession of a valid driver's license and dependable transportation.
Physical Requirements:
- Ability to operate a vehicle.
- Capability to navigate stairs.
Reporting Structure:
Reports directly to the Program Manager.
Work Environment:
- Interaction with clients across various programs.
- Successful completion of a background check is mandatory.
