Key Responsibilities:

  • Administer case management services within the emergency shelter environment.
  • Collaborate with clients to create tailored service plans.
  • Provide ongoing support and follow-up as clients engage with new services.

  • Ensure accurate maintenance of client records and documentation.
  • Engage in team meetings and maintain effective communication.
  • Uphold client confidentiality at all times.
  • Be prepared to cover shifts during emergencies.

Qualifications and Skills:

  • Bachelor's degree or a minimum of three years of relevant experience.
  • Awareness and sensitivity to diverse low-income populations.
  • Excellent listening and communication abilities.
  • Capability to work autonomously.
  • Bilingual skills are advantageous.
  • Possession of a valid driver's license and dependable transportation.

Physical Requirements:

  • Ability to operate a vehicle.
  • Capability to navigate stairs.

Reporting Structure:
Reports directly to the Program Manager.

Work Environment:

  • Interaction with clients across various programs.
  • Successful completion of a background check is mandatory.
