Job Description:

We are seeking a skilled and compassionate Registered Nurse to join our team at the Regional Medical Center of Memphis as a Nurse Care Coordinator. In this role, you will have the opportunity to provide exceptional patient care and work collaboratively with our interdisciplinary team to deliver high-quality healthcare services.

About the Role:

As a Nurse Care Coordinator, you will be responsible for assessing patients' physical and emotional needs, developing personalized care plans, and implementing interventions to promote optimal health outcomes. You will also collaborate with patients, families, and healthcare providers to ensure seamless transitions of care and continuous communication throughout the patient journey.

Requirements:

To be successful in this role, you must possess a graduate degree from an approved nursing program and hold a current RN licensure in the state of Tennessee. A minimum of one year of recent experience as a Registered Nurse is required, with a focus on geriatric or adult populations preferred. Additionally, you should demonstrate excellent clinical skills, strong communication abilities, and a commitment to delivering patient-centered care.

Physical Demands:

This position requires prolonged standing and walking, lifting, turning patients and equipment, repeated bending, squatting, and stooping, exposure to infectious agents, and visual acuity to assess color changes, verify accuracy of written materials, and administer medications. Heavy Work is required, exerting up to 150 pounds of force occasionally, and/or up to 75 pounds of force frequently, and/or up to 30 pounds of force constantly to move objects.

Salary and Benefits:
