Job Description

We are seeking a highly skilled and compassionate Nurse Coordinator to join our team at DaVita Inc.

About the Role

This is an exciting opportunity to work as part of our dialysis care team, providing comprehensive patient care in an independent setting. The successful candidate will be responsible for coordinating the ongoing care of each patient, ensuring their dialysis treatment stays on track.

Responsibilities
  • Provide direct patient care, including supervising Dialysis Technicians and delivering optimal dialysis care.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Partner with SNF staff to coordinate care and provide patient education.
Requirements

To be considered for this role, you must have:

  • A current Registered Nurse (RN) license in the state of practice.
  • A minimum of 12 months' experience as a RN.
  • Prior experience in chronic hemodialysis, acute dialysis, or peritoneal dialysis nursing.
  • An Associate's Degree in Nursing (A.D.N) from an accredited school of nursing.
