Job Title: Licensed Practical Nurse Care Coordinator

We are seeking a skilled and compassionate Licensed Practical Nurse to serve as a Care Coordinator in our team. As a key member of our patient support services, you will play a crucial role in delivering exceptional care and support to patients and their families.

About the Role:

  • Cultivate strong relationships with patients, caregivers, and healthcare providers to ensure seamless care coordination.
  • Provide education and support on therapeutic areas, medication adherence, and treatment options.
  • Monitor patient progress, identify potential issues, and develop strategies to address them.
  • Maintain accurate records and reports to meet regulatory requirements and program objectives.

Requirements and Qualifications:

  • Bachelor's degree in Nursing or related field (Associate's degree with relevant experience acceptable).
  • Current licensure as a Licensed Practical Nurse in Tennessee.
  • Minimum 2 years of nursing experience, preferably in a similar role.
  • Excellent communication and interpersonal skills.
  • Able to work in a fast-paced environment and prioritize multiple tasks effectively.
  • Familiarity with electronic health records (EHRs) and medical software.

Estimated Salary: $62,000 - $72,000 per year
