We are seeking a compassionate and skilled Hospice Aide to join our team at Community Hospice & Palliative Care. As a Hospice Aide, you will play a vital role in providing high-quality patient care and support to individuals with terminal illnesses.
Key Responsibilities- Provide personal care and assistance with daily living activities, including bathing, dressing, and grooming.
- Assist patients with medication management and provide education to family members and caregivers.
- Participate in interdisciplinary team meetings to discuss patient care plans and goals.
- Document patient care and progress in accordance with agency standards.
- Maintain patient confidentiality and adhere to HIPAA guidelines.
- High school diploma or equivalent required; Certified Nursing Assistant (CNA) certification in the state of Florida preferred.
- At least six months of experience in home care, hospital, or long-term care settings; experience in caring for the terminally ill a plus.
- Current State of Florida driver's license and reliable transportation.
