Hospice Care Professional Opportunity
We are seeking a skilled Clinical Specialist to join our team at Haven Hospice, a leading provider of hospice care services. As a Clinical Specialist, you will play a critical role in delivering high-quality patient care and ensuring the smooth operation of our hospice services.
Key Responsibilities:
- Provide clinical expertise in the nursing management of patients, documentation, and care planning.
- Process start of care workflow once the start of care visit has been completed.
- Participate in Interdisciplinary Group (IDG) meetings.
- Review visits plotted on Start of Care (SOC) or Re-cert visit and adjust as necessary.
- Maintain orders in the Hospice Common Hospice Billing (HCHB) system.
- Manage admissions, deaths, discharges, and assist with these processes to ensure proper documentation is completed.
- Assist in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned.
- Uphold effective communication with referral sources to ensure smooth transition of admissions.
Requirements:
- Associate degree in Nursing with current license.
- Three years of practice as a Registered Nurse.
- Previous hospice experience preferred.
- Knowledge of the Hospice Philosophy of Care.
- Ability to use Microsoft Word, Excel, Outlook, and other software programs.
