Hospice Care Specialist Role

We are seeking a skilled Clinical Specialist to join our team at BrightSpring Health Services. As a Clinical Specialist, you will play a vital role in providing high-quality care to patients and their families in a hospice setting.

Key Responsibilities:

  • Provide clinical expertise in nursing management, documentation, and care planning for patients.
  • Process start of care workflow and participate in Interdisciplinary Group (IDG) meetings.
  • Review visits plotted on SOC or Re-cert visit and adjust as necessary.
  • Maintain orders in HCHB and manage admissions, deaths, and discharges.
  • Assist with data collection for compliance, quality assurance, and staff development.
  • Uphold effective communication with referral sources to ensure smooth transition of admissions.

Requirements:

  • Associate degree in Nursing with current license.
  • Three years of practice as a Registered Nurse.
  • Previous Hospice experience preferred.
  • Knowledge of the Hospice Philosophy of Care.
  • Ability to use Microsoft Word, Excel, Outlook, and other software programs.
