The ideal candidate will act as a liaison and representative of the residents' social interests, assisting in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department.
Key Responsibilities:
- Develop a comprehensive social assessment and care plan to identify medically related social and emotional problems and needs.
- Participate in pre-admission and admissions processes to ensure a smooth transition for residents.
- Provide individual and group support to residents and families during times of adjustment, crisis, or need.
- Document residents' behavioral and psychosocial status to inform care decisions.
- Establish and maintain volunteer programs to meet specific social service needs.
- Orient residents and families to the nursing center, facilitating a successful adjustment to the new environment.
Qualifications:
- Bachelor's Degree in Social Work or a related field from an accredited institution.
- Knowledge of social services practices, procedures, laws, regulations, and guidelines governing long-term care.
