MDS Assessment Coordinator

Rose City Nursing and Rehab is seeking a skilled professional to fill the role of Registered Nurse Assessment Coordinator. This position is responsible for conducting and coordinating the development and completion of resident assessments in accordance with state requirements and facility policies.
Key Responsibilities:- Routinely assess residents according to pre-set schedules or as needed for condition changes, hospital return, etc.
- Ensure accurate observation, assessment, and communication of condition changes to appropriate personnel.
- Assure completion of assessments and care plans by other departments, such as Social Services, Activities, and Dietary, and obtain corresponding signatures.
- Complete other sections of MDS, quarterlies, and care plans in the absence of other personnel.
- Initiate care plans based on resident needs identified in the Resident Assessment Protocols and update care plans according to guidelines.
- Prepare and distribute MDS schedules to the interdisciplinary team.
- Transmit MDS to the state weekly and maintain accurate records, including a transmittal log.
- Registered Nurse RN of this state.
- Over a year's experience in Long-Term Care as an MDS assessment coordinator.
