Job Title: MDS Assessment Coordinator

Rose City Nursing and Rehab is seeking a skilled and experienced Registered Nurse to join our team as an MDS Assessment Coordinator. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality of care for our residents.

Job Summary:

The primary purpose of this position is to conduct and coordinate the development and completion of the resident assessment in accordance with state requirements and facility policies. You will be responsible for accurately assessing residents, communicating condition changes to appropriate personnel, and ensuring the completion of assessments and care plans.

Responsibilities:
  • Routinely assess residents according to pre-set schedules or as needed for condition changes, hospital returns, etc.
  • Responsible for accurate observation, assessment, and communication of condition changes to appropriate personnel.
  • Assure completion of assessments and CAA's by other departments such as Social Services, Activities, and Dietary, and obtain corresponding signatures.
  • Complete other sections of MDS, quarterlies, CAA's, and care plans, etc., in the absence of other personnel.
  • Initiate care plans based on resident needs identified in the Resident Assessment Protocols and update care plans according to guidelines.
  • Prepare and distribute MDS schedule to interdisciplinary team.
  • Transmit MDS to the state weekly (or as required) and maintain accurate records, including a transmittal log.
Qualifications:
  • Registered Nurse (RN) of this state.
  • Over a year's experience in Long-Term Care as an MDS Assessment Coordinator.
