New York, New York, United States Public Health Solutions Full time
Job Overview

Company Background:

Public Health Solutions (PHS) is a distinguished 501(c)3 non-profit organization dedicated to enhancing health equity and addressing health-related social determinants for historically marginalized communities. As the foremost public health nonprofit in New York City, we strive to improve health outcomes and empower communities by delivering essential services to vulnerable families, collaborating with community-based organizations through robust public-private partnerships, and bridging the divide between healthcare and community services. Our focus encompasses a broad spectrum of public health challenges, including nutrition, health insurance, maternal and child health, sexual and reproductive health, tobacco control, and HIV/AIDS.

PHS oversees WholeYouNYC (WYNYC), a coordinated community resource network that establishes reliable connections between healthcare providers, health plans, and community-based organizations, delivering vital resources that address the social determinants of health. WYNYC unites over 100 organizations providing various programs, such as food, housing, employment, health insurance, and sexual health services, across all five boroughs. These initiatives enable New Yorkers to achieve optimal health and significantly reduce health disparities while advancing health equity.

Recently, New York State announced a substantial funding opportunity to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery across various regions. Public Health Solutions (PHS) and our WYNYC network have been designated as the regional SCN for Brooklyn, Manhattan, and Queens.
