Key Responsibilities:
  • Complete MDS assessments, CAAs, and care plans within required timelines.
  • Coordinate the RAI process with the interdisciplinary team.
  • Conduct resident assessments through physical evaluations, interviews, and chart reviews.
  • Collaborate with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activities personnel to address resident care needs.
  • Review information from hospitals and outside agencies to inform assessments and care planning.
  • Identify and initiate significant changes in MDS as necessary.
  • Prepare for and conduct PPS meetings, ensuring MDS assessments are maintained according to Medicare schedules.
  • Stay updated with the requirements set by the American Association of Nursing Assessment Coordinators (AANAC).

Qualifications:
  • Current Licensed Practical Nurse (LPN) license.
  • AANC certification is a plus; RAC-CT preferred.
  • Strong knowledge of the Resident Assessment Instrument (RAI) process and the Prospective Payment System (PPS).
  • Previous experience as an MDS Nurse is highly desirable.
