We are seeking a highly motivated and compassionate Day Habilitation Specialist to join our team at ACHIEVE. As a Direct Support Professional, you will play a vital role in promoting independence and support for individuals with moderate to severe disabilities.
Key Responsibilities- Implement goal plans and data collection to monitor progress.
- Transport individuals to volunteer and recreational activities in the community, providing necessary supervision, training, and support.
- Maintain accurate daily participant records, charts, logs, and case/progress notes.
- Ensure a safe and clean environment at all times.
- Order supplies with approval from the Direct Supervisor.
- Assist individuals with toileting, if needed.
- Complete monthly progress notes, semi-annual, and annual reviews as assigned.
- Assist in boarding vehicles and disembarking from vehicles, maintaining orderliness and passenger safety.
- Plan activities that foster community integration.
- High School Diploma or GED.
- Possess a valid driver's license acceptable to the agency's insurance carrier.
We provide all training, which is paid and provided. No experience is necessary. We offer a competitive salary of $16.00-$17.25 per hour.
