Responsibilities of the LPN Care Specialist:

  • Provide one-to-one patient care in a home environment, ensuring the highest level of comfort and dignity for our patients.
  • Execute physicians' prescribed plans of care and maintain accurate documentation of care provided.
  • Administer prescribed medication, treatments, and therapies as ordered by the physician.
  • Conduct patient assessments and coordinate care with other healthcare professionals as needed.
  • Provide health education and training to patients and their families, promoting healthy habits and disease prevention.
