About the Role:

The Elizabeth Hospice is seeking a skilled LVN to join our team in providing high-quality care to patients in their homes or facilities. As a Continuous Care LVN, you will work under the supervision of an RN to provide professional nursing care to patients across the lifespan, including perinatal, pediatric, and adult patients.

Responsibilities:

  • Assist the RN in analyzing situations, anticipating problem areas, and taking appropriate action.
  • Evaluate the response of patients to care, ensuring safe and timely outcomes.
  • Perform licensed nurse skills, adhering to policies and procedures, standards of care, and critical competencies.
  • Implement and document teaching plans based on patient and family needs, in a timely and culturally sensitive manner.
  • Document all care provided, including telephone interactions with other staff members, using electronic charting.
  • Enter or retrieve data from established computer files.
  • Orient, instruct, and train assigned personnel.
  • Perform phlebotomy as needed.
  • Provide support to the agency in completing assigned audits and office duties.

Requirements:

  • Vocational Nurse Training.
  • Minimum 12 months of experience as an LVN, with preferred experience in acute care settings, home health, or hospice.
  • Current California LVN license.
  • Familiarity with computerized bedside patient documentation.
  • Ability to function as part of a team and take direction from other team members.
  • Phlebotomy certification a plus.
  • Bi-lingual (Spanish) a plus.
  • Current California driver's license and proof of insurance required.
  • Excellent communication skills with patients, families, and medical professionals.
  • Ability to remain calm and flexible in stressful situations.
  • Current CPR/BLS Certification required.
  • Current TB test results required.
