Hospice LVN 3pm Position

As a Licensed Vocational Nurse, you will play a vital role in our team, providing direct patient care and working collaboratively with our multidisciplinary team to achieve positive patient outcomes.
Key Responsibilities- Provide direct patient care under the supervision of a registered nurse, following the plan of care and implementing treatments as needed.
- Work collaboratively with the team to identify and address patient needs, ensuring timely and effective care delivery.
- Assist in the development and implementation of teaching plans, tailored to the individual needs of patients and families.
- Document all care provided, including telephone interactions with other staff members, in accordance with charting standards.
- Enter or retrieve data from established computer files, either at the site of care or remotely.
- Orient, instruct, and train assigned personnel, ensuring seamless handovers and effective knowledge transfer.
- Support the agency in completing assigned audits and other office duties as required.
- Be cross-trained in Clinical, Triage, Admissions, Access Center, and as a Program Representative.
- Vocational/Specialized Training
- Current California LVN license
- Ability to answer phones, maintain patient charts, and schedule appointments
- Intermediate proficiency in Office applications and EMR system
- 3 years of clinical experience, with a focus on hospice care
- Licensed driver with an insured automobile
