Hospice Traige LVN Job Description

Job Summary:

The Elizabeth Hospice is seeking a skilled Licensed Vocational Nurse (LVN) to join our team in providing high-quality care to patients in their homes or facilities. As an LVN, you will work under the supervision of a Registered Nurse (RN) to provide professional nursing care to patients across the lifespan, including perinatal, pediatric, and adult patients.

Responsibilities:

  • Assist RN in analyzing situations, anticipating problem areas, and taking appropriate courses of action.
  • Evaluate patient responses to care and make adjustments as needed.
  • Perform licensed nurse skills, including phlebotomy, as stated in basic skills checklists and policies.
  • Implement and document teaching plans based on patient and family needs.
  • Document all care provided to patients, including telephone interactions with other staff members.
  • Enter or retrieve data from established computer files.
  • Orient, instruct, and train assigned personnel.
  • Perform other duties as assigned.

Requirements:

  • Vocational Nurse Training.
  • Minimum 12 months of experience as an LVN, with preferred experience in acute care settings, home health, or hospice.
  • Current California LVN license.
  • Familiarity with computerized bedside patient documentation.
  • Ability to function as part of a team and take direction from other team members.
  • Phlebotomy certification a plus.
  • Bi-lingual (Spanish) a plus.
  • Current California driver's license and proof of insurance required.
  • Excellent communication skills with patients, families, and medical professionals.
  • Ability to remain calm and flexible in stressful situations.
  • Current CPR/BLS Certification required.
  • Current TB test results required.
