Hospice Visions Inc. is seeking a compassionate Hospice Home Care Professional to join our team.
Estimated Salary: $65,000 - $90,000 per year
About the RoleThis is a full-time position that requires working under the supervision of an RN Case Manager to provide quality hospice care in a home setting.
Main Responsibilities:
- Assess and manage patient pain and discomfort according to established standards of nursing practice.
- Support patients and their families during times of crisis and uncertainty.
- Document patient information accurately and timely using the nursing process.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
To be successful in this role, you will need:
- A current license to practice as a licensed professional nurse in Idaho.
- Valid Idaho Driver's License and availability of a car.
- Strong analytical and problem-solving skills.
- Ability to work effectively in a dynamic and ever-changing environment.
