Key Responsibilities:
  • Conduct thorough interviews with patients to assess their needs and document case activities.

  • Foster and maintain relationships with local health, welfare, and social agencies to creatively address patient needs.

Qualifications:
  • Master's degree in Social Work.
  • No prior experience is necessary.

Physical Requirements:
Frequent activities include reaching, sitting, standing, and walking. Vision and depth perception are essential for tasks at varying distances. Effective communication skills are required for sharing information and responding to alarms or notifications. The role may involve occasional lifting and carrying of light objects.

Work Environment:
