The Clinical Social Services Specialist plays a vital role in our healthcare system by ensuring that patients transition safely from the hospital. This includes identifying potential obstacles and collaborating with both the patient and the care team to address these challenges.

Key Responsibilities:

  • Provide social work support to patients and their families concerning social determinants of health.
  • Conduct patient interviews, prioritize needs, and document case activities.
  • Collaborate with the multi-disciplinary team and community resources to facilitate patient treatment.
  • Assist patients and families in accessing community support through appropriate referrals.
  • Establish and nurture relationships with community health, welfare, and social agencies to creatively meet patient needs.

Qualifications:

  • Master's degree in Social Work.
  • Frequent use of keyboard for data entry.
