Position Overview

The Geriatric Social Services Specialist plays a crucial role in delivering targeted and individualized group and/or one-on-one sessions tailored to meet the social service requirements of our residents. This position is essential in addressing the concerns of residents and their families, serving as a supportive resource for those transitioning into a long-term care setting.

Key Responsibilities:
  • Maintain availability to residents during times of need, assisting with adjustments, lost belongings, and other concerns.
  • Engage in the planning and execution of effective, personalized social service programs through thorough assessments, interdisciplinary collaboration, and meticulous documentation.
  • Ensure accurate charting and uphold resident rights through diligent observation and assessment, reviewing advance directives upon admission and annually thereafter.
  • Collaborate with the interdisciplinary team to address the needs of residents and their families within the scope of practice.
  • Foster relationships with residents and their families, addressing their concerns in a professional and timely manner.
  • Conduct social service assessments upon admission and for residents returning from hospitalization.
  • Assist residents' representatives with correspondence, documentation, and Medicaid applications as needed.
  • Coordinate and schedule appointments for residents with healthcare providers and other necessary services.
  • Participate in the development of admission, quarterly, and annual notes to support residents' physical and emotional well-being.
  • Contribute to Resident Care Plan meetings, documenting residents' strengths, weaknesses, and social service needs to inform care planning.
  • May undertake additional responsibilities as required.


Qualifications
  • High school diploma or equivalent required.
  • Bachelor's degree in Social Work preferred.
  • Knowledge of growth and development, particularly in the geriatric population, is essential.
  • Certification as a Social Service Designee by the State is preferred.
  • Minimum of one year of experience in a long-term care social service department is preferred.
  • Previous experience in geriatric care and social work is advantageous.
  • Proficient in both written and electronic documentation.
  • Demonstrated manual dexterity and skillful use of hands when interacting with patients and equipment.
